Egad – I can’t get the picture to resize, sorry!

Figure out about how much text you have, then figure out how large you want the labels at the bottom to appear. Make sure you have some clearance, then set up your page dimensions in word to that size. Select two columns and ensure that your center margin is 2x your side margin, that way you can cut your paper in half and still have everything centered.
Do a lot of trial prints, to make sure things are where you want them. Remember that by the bottom layer, you can’t print right at the top because the pages above will be covering a bit of the top when you lift them up.
I did not glue things, only used the ribbon and hole punch. I re-set my 3 hole punch in order to do the top two holes. The fold over piece is slightly longer on the back and I ended up using a scoring blade to create an 1/8th inch or so fold at the top, to contain the pages better.
This project takes a million hours on the paper cutter, be warned!
